 |  |   | Blue Roster Form & Instructions |  MASSACHUSETTS STATE CUP ROSTER Revised 8/24/07 Instructions and Roster Form A copy of the required roster for participation in all State Cup competition events is available at the end of these instructions. By following these instructions, you will be able to complete the roster on line and email the form to the Massachusetts Youth Soccer office. Please read these instructions carefully. Failure to follow the instructions will result in the roster being returned to you without the approval from Mass Youth Soccer, and will constitute an invalid roster for any and all State Cup competition matches. HELPFUL HINT: PRINT THESE INSTRUCTIONS TO USE AS A GUIDE BEFORE YOU COMPLETE YOUR ROSTER. Note: When completing the roster form, please use the mouse to locate your spot on the form. Do not tab. - Click on the State Cup “Blue” Roster” at the end of these instructions.
- Once the form has opened, save as a Word document in a file of your choice.
- Close the web site. Open Word and then open the saved file containing the Cup roster.
- On the top portion of the form, type all requested information. Enter your team(Club) name, i.e. FC Greater Boston, your age group and put an "X" in the box next to either Boys or Girls.
- Enter Coach's name, home and cell number and complete address.
- Enter names of any Assistant Coaches or Trainers.
- Enter Team Manager's Name, home and cell number, and complete address.
- In the section dealing with player information:
Fill in the "JERSEY #" column with each player's primary uniform number. List players in alphabetical order, last name first. The player's name should appear exactly as it appears on the player pass card. If a middle initial is used on the pass card, it must appear on the roster. If the player's full first name is used on the pass card, it must be used on the roster (for example, James, not Jim; Christine, not Chris). If a suffix is shown on the pass card (e.g. “Jr.”) it must appear on the roster. Remember to type last name, then first name, then middle initial and/or suffix if applicable. Be sure to leave the “Signature” and “Registration Number” columns blank. Massachusetts does not use registration numbers and players DO NOT sign the roster until check-in at the State Cup Finals. The player's birth date should be typed as “mm/dd/yy”. For example, July 19, 1985 will appear as 07/19/85. ALT #: Enter the uniform number of the player's alternate color jersey. Game Active: Applies to U17, U18 and U19 ONLY. A copy of the Blue Roster will become the game roster for each game. The team will check this block on the game roster to indicate which players will be participating in the match.
- At the bottom of the roster form, in the space reserved for Coach or Manager Signature, the coach or manager should type their name. The downloaded form willl automatically convert the typed signature to cursive. Type in the date.
- After you are done, please proofread the form checking for typos, etc.
- Save the completed roster.
- Email completed roster AS AN EMAIL ATTACHMENT and IN MS WORD FORMAT to Kate Murphy at the Mass Youth Soccer office: kmurphy@mayouthsoccer.org
- The Mass Youth Soccer office will review the completed roster, will affix the appropriate signature and return the roster to you. You can then print this roster for your competition matches.
