ATTENTION:   MANDATORY CORI/Kidsafe form to be submitted by ALL Mass Youth Adult members!  If you have submitted the cori background information form after June 2005 - there is no need to submit a new one.  any one who has not submitted the cori background form or is a new  coach, administrator, OR referee  MUST submit the form

IMPORTANT CHANGES TO REGISTRATION PROCEDURES

Registration Report Deadlines. Your first Spring registration report, which should include all required forms and fees, must be submitted to the Mass Youth Soccer Office no later than Wednesday, July 22nd 2009. Your final registration report for the Spring season will be due Friday, October 2nd.  Your early compliance with the registration deadline dates is appreciated, and the first reporting deadline is critical from the standpoint of ensuring that all players and coaches are covered by Mass Youth Soccer's insurance coverage by the time practice begins. No player or coach should be practicing or playing prior to affiliation with Mass Youth Soccer.

NEW – Beginning with this season we are requiring that you submit your file via an encrypted folder.  The information shown below was previously forwarded to all presidents and registrars.

REGISTRATION SUBMISSION UPDATE!!!

Over the past year or so, we have had a number of inquiries about the security of our registration files.  Since most clubs are sending these to us via the internet, we have a responsibility to ensure that personal data is protected to the greatest extent possible.  Toward that end we have made the decision to require that the files come to us in an encrypted format.  This ensures a much higher degree of certainty that the files are protected.

We will give all clubs two options to accomplish this.

Option 1.   SSL.  Secure Sockets Layer (SSL) technology.  An SSL enables encryption of sensitive information during online transactions.
This option will require no additional work on your part and well ensure your data is more secure than just emailing us a file.  You will access this option via our web site, www.mayouthsoccer.org .  After selecting Registrations you will find the “submit registration files” option.  There you will find the instructions and another link to the SSL.  

Option 2.  For those who want an additional level of security, we will direct you to our technology consultant who will walk you through the process.  This is slightly more involved, requires the downloading of some free software, and the use of something called a “public key” which we will provide.  This method is more secure than SSL, however either method will be a significant upgrade over our current process.

If you want to use option 2, please contract Rachel Woo and she will direct you to Dave Wheeler, our consultant.

If you have ever been a victim of identity theft, you will understand why we are instituting these measurers.  We want to take every reasonable step to protect your data. 

When you send this information to the Mass Youth Soccer office, please include your club's name in the email subject line.  We would also appreciate it if you would send an e-mail to Rachel Woo advising her that the file has been sent.  The Fee Submission Form can be saved and sent as an attachment to rwoo@mayouthsoccer.org.  If you are a new registrar and need assistance in the submission process, please contact Rachel (ext. 236 or rwoo@mayouthsoccer.org) at the Mass Youth Soccer Office.

Legislation has made it mandatory that a CORI background form must be completed for every adult volunteer.  If you completed this form after June of 2005 and you have not changed your last name, you do not need to complete another form at this timeNEW -- The adult name must be shown as the legal given name.  Please refrain from using a nickname.  This means that your registration file must have also have the legal name shown for all adult/coach volunteers.  DATE OF BIRTH IS MANDATORY FOR ALL AFFILIATES.  No adult will be considered registered unless a correct date of birth is in your file.

You are reminded that the affiliation fee is $11.00 per player and $11.00 per adult.  The 09-10 fee submission form reflects this fee.  You are reminded that you must affiliate all recreational and in-town players, coaches, assistant coaches, managers, administrators and board members as well as other volunteers who assist the program, etc. as well as those involved with travel and premier teams.

Please note that the required Certification Statement is now included in the Fee Submission Form.

Pass Cards are available through our website.  Membership forms are no longer available as a paper form; you may print out copies from the .pdf on our website here.
